Please call our office if you do not see your insurance on the list above. In the event, we do not accept your insurance, some or all of your fees may still be covered because your insurance may pay a percentage of your fee at an out of network rate. In this case, you will be responsible for 100% of the fee at the time of your appointment. We will give you a form, called Superbill, to send to your insurance and they may reimburse you directly for your covered percentage (please check with your insurance if you have an out of network coverage).
